Remote Care Registration
MED-EL's Remote Care allows you to support your cochlear implant patients at any time that's convenient for you.
Getting started with Remote Care:
- Complete the form below and click "Begin Remote Care Registration" to begin the Remote Care enrollment process for your center.
- A packet of documents will be delivered via DocuSign to the email address designated below.
- Have the appropriate person(s) at your center review and sign the HIPAA Addendum and License Agreement.
- Complete and submit the Remote Care Authorization Form for your center.
- Upon receipt of completed and signed documents, MED-EL will enable Remote Care for authorized clinicians at your center.
We kindly ask that you submit only one (1) Remote Care Registration per center. Please coordinate with your team to avoid duplicate registration requests.
